DASv Startup Request Acknowledgement
Be sure to review your check list and be certain that your responses and data are
complete and accurate. This is absolutely required to ensure a timely repose to your
request and schedule. Be assured that our goal is to furnish you with the best and
most thorough service in.
By signing you acknowledge the accuracy and completeness of your
submission and agree that if the equipment listed in your request IS NOT prepared
for startup as you have indicated that our service technician cannot perform or
complete the startup. This will make your project subject to the conditions listed
below.
- Our service tech will mark the report as incomplete and list cause(s) for
such and leave the site. Only features in the unit that impact fan(s),
compressor(s), space heating, reheat systems and air conditioning will be
considered critical and therefore cause to not complete the startup.
- You will receive a copy of the site report describing the insufficiencies.
- All incomplete installation items must be completed before you resubmit
your checklist.
- The return visit is required to complete the startup and activate
the unit warranty coverage. This warranty will be forfeit if the
unit startup remains incomplete.
- Your request will be rescheduled in the order it is received depending on
availability in our scheduling process.
- The return fee to complete the startup must be paid for before the return
date will be scheduled.
